Dear Department of Environmental Protection Employees:
Richard Smith & Associates, Inc is pleased to announce that we will be offering an opportunity for you to participate in the Reliance Standard Voluntary Group Term Life Insurance program through a special enrollment. This enrollment period will be held from February 15 through March 31, 2010 with an effective date of April 1, 2010.
So, what does this mean for you and your family? You now have an opportunity to purchase additional Life Insurance at very affordable group rates. The following are the enrollment guidelines.
During this special enrollment period if you have not previously enrolled in the plan, you may apply for employee Voluntary Group Term Life coverage up to the Guaranteed Issue amount of $100,000.00 as long as you are under the age of 60. If you are age 60 to age 70 then your Guaranteed Issue Amount of Coverage is $10,000.00.
For the first time, our Voluntary Group Term Life Insurance program is offering a Guaranteed Issue amount of $20,000.00 for spouses that are under the age of 60 and this coverage can be elected without the employee electing any amount. This coverage will be issued without answering any medical questions up to the guaranteed issue.
You may also purchase Term Life Insurance for your dependent child(ren) in amounts of either $2500, $5000, $7500 or $10,000. This coverage will be issued without answering any medical questions. Please note that to insure your dependent child(ren) you must be enrolled under the employee Voluntary Life coverage and/or the spouse Voluntary Life coverage.
If you are currently enrolled in the Voluntary Group Term Life plan, you have the opportunity to increase your coverage in increments of $10,000 up to the policy’s Guaranteed Issue amount without answering medical questions. You will need to answer medical questions if you want coverage above the Guaranteed Issue amount.
Any employee who has previously been denied coverage, voluntarily terminated their coverage or withdrew their request for coverage will be required to answer medical questions for any elected amount of coverage.
