SUBJECT:            Family Group Term Life Insurance

The voluntary group term life insurance plan, underwritten by Reliance Standard Life Insurance Company, will have an OPEN ENROLLMENT beginning February 1 through February 29, 2008.

This insurance is available to all active full-time employees who are working at least 30 hours per week. 

This plan has the following important features:

1.                Insurance coverage is available to your spouse and children.

2.                Availability of a “living benefit rider” equal to 50% of the death benefit, to a maximum of $250,000, if you become terminally ill and have met all requirements.

3.                If you have been insured for 6 months, are under age 60, and you have reached the $100,000.00 guaranteed issue limit, you may purchase an additional $10,000.00 at this time; and an additional $10,000.00 at each future enrollment up to a maximum of $50,000 over a 5 year period.

4.                The maximum GUARANTEED amount of coverage for employees, available without evidence of insurability during the annual enrollment, is $10,000.00.  The maximum amount of coverage an employee or spouse may purchase with evidence of insurability is $500,000.00.

5.                If you leave the Department you may keep this insurance or convert to an individual permanent life insurance policy.
