SUBJECT: OPEN ENROLLMENT -VOLUNTARY GROUP TERM LIFE INSURANCE PLAN

Employees have another opportunity to enroll in our Voluntary Group Term Life Insurance Program offered by Reliance Standard Life Insurance company through open enrollment beginning April 1, 2008, and extending through May 30, 2008.

What benefits are available? During the open enrollment period employees may apply for up to $10,000 Guaranteed Issue (GI) coverage if under age 60. Employees and their spouses may apply for maximum coverage up to $500,000 each; however amounts in excess of the GI limit must be medically approved by Reliance. Dependent children may be covered for up to a maximum of $10,000, medical underwriting is not required. Benefits are 100% portable after employment terminates. How do I obtain information? Enrollment packets with brochures and enrollment forms will be distributed to each Human Resources Office Statewide. If you have any questions, please contact Lois Goode with Richard Smith and Associates, Inc. toll free at (800)342-0209 or (850)877-1445.

Who is eligible? All active full-time employees who are working a minimum of 30 hours per week. Spouse coverage will be issued up to age 70. Unmarried eligible dependent children from 14 day to 20 years of age, or to age 25 if full-time student, may be insured.

How do I enroll? Employees and their spouses will have an opportunity to apply for Voluntary Group Term Life Insurance through an open enrollment period beginning April 1, 2008 through May 30, 2008. To enroll, please take a moment to read the brochure in its entirety and complete the application enclosed in the enrollment packet. Deductions will begin after your application is reviewed and approved by Reliance, however, if the amount applied for is below or up to the guaranteed issue limit, coverage becomes effective once the application is signed. All other applications are subject to medical approval.
